GCRC Encounter Forms
On August 8th, 2001 the NIH announced (http://grants.nih.gov/grants/guide/notice-files/NOT-OD-01-053.html)
that it has adopted the 1997 Office of Management and Budget (OMB) revised
minimum standards for maintaining, collecting, and presenting data on
race and ethnicity for all active research grants. The minimum standards
are described in the 1997 OMB Directive 15 (http://www.whitehouse.gov/omb/fedreg/ombdir15.html).
The biggest change has been the shift of data previous collected under
race into the new ethnicity category. This will change the way the GCRC
collects and reports ethnicity and race information to the NIH. New encounter
forms have been created and will need to be used after December 1, 2001.
People may leave questions blank or may choose more than one option for
each question.

The OMD definitions of the categories are as follows:
American Indian or Alaska Native - A person having origins in
any of the original peoples of North and South America (including Central
America) and who maintains tribal affiliation or community attachment.
Asian - A person having origins in any of the original peoples
of the Far East, Southeast Asia, or the Indian subcontinent including,
for example, Cambodia, China, India, Japan, Korea, Malaysia, Pakistan,
the Philippine Islands, Thailand, and Vietnam.
Black or African American - A person having origins in any of
the black racial groups of Africa. Terms such as "Haitian" or "Negro" can
be used in addition to "Black or African American."
Hispanic or Latino - A person of Cuban, Mexican, Puerto Rican,
Cuban, South or Central American, or other Spanish culture or origin,
regardless of race. The term "Spanish origin" can be used in
addition to "Hispanic or Latino."
Native Hawaiian or Other Pacific Islander - A person having origins
in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific
Islands.
White - A person having origins in any of the original peoples
of Europe, the Middle East, or North Africa.
